You can rest assured knowing that you’re well taken care of at Avera. However, if you would like to speed up the process, you can print the DOT physical form and bring it to your appointment with your information and health history completed.
At the conclusion of your physical exam, you’ll receive the motor carrier copy of the medical examiner’s certificate to send to your employer.
