Billing & Insurance
It is the patient’s responsibility to provide the business office with accurate insurance policy information. Bring your insurance, Medicaid and/or Medicare card to your appointment. We will submit claims to your insurance company on your behalf, however, the account balance is ultimately the patient’s responsibility.
All co-payments and deductibles are due at the time of service. For patients without insurance, payment plans are available. A Patient Payment Information form is available online.
If you receive care for a work-related injury, please notify the receptionist, as we will need to obtain employer information for billing purposes.
