Finding Us: Our facility is located in the 300 Building, one block north of the downtown Sioux Falls Public Library. Free parking is available next to the building.

This Avera primary care clinic in downtown Sioux Falls, SD, provides free medical care to underinsured patients from board-certified physicians and advanced practice providers. For more than 20 years, personalized health care services have been available for patients of all ages who do not have insurance, Medicare or Medicaid.

Primary Care Services

Yearly Checkups

Yearly checkups allow you and your primary care provider to build a trusted relationship around your health — whether that’s physical, mental or emotional health. During this annual doctor’s visit, you and your provider might discuss your:

  • Current health status
  • Immunizations
  • Health history
  • Potential health risks
  • Preventive screenings you may need
  • Personal health goals
  • Current prescriptions, over-the-counter drugs, vitamins or minerals you’re taking

This appointment is also the time to bring up any concerns or changes you’ve noticed so that your provider can help you take the next steps in care. If you’re a parent, a yearly child checkup can also count as a sports physical, but a physical is not a substitute for a checkup.

Sports Physicals

Getting a sports physical is often required of athletes before the season begins. Many primary care providers are trained in assessing and establishing a baseline in athletic physical health. If an injury occurs, the physical can help navigate progress during recovery.

Schedule your child’s sports physical as soon as possible. Our pediatric specialist at this location often has a waiting list. Sports physicals are offered every Tuesday, 5 – 6:30 PM

Disease Management

Some diseases require ongoing medical attention and lifestyle changes. Avera primary care experts can help by prescribing treatments including medications, providing insight on effective lifestyle changes, and supervising progress under the direction of specialized experts.

Chronic conditions include, but aren’t limited to:

  • Alzheimer’s disease and other dementias
  • Asthma
  • Arthritis
  • Diabetes type 1 and 2
  • Crohn’s disease
  • High blood pressure
  • Irritable bowel syndrome
  • Osteoporosis

Immunizations

Staying up to date on your immunizations is an important step you can take to prevent illness. Children from newborn to 18 who don’t have health care coverage can receive immunizations free of charge at this clinic. School immunizations are offered weekdays, 9:30 AM – 3:30 PM. Your primary care provider can inform you of what vaccines your child needs.

Screenings for Women

Women between the ages of 30 and 64 who are eligible for the South Dakota Department of Health All Women Count! Program can take advantage of screenings for breast cancer and cervical cancer free of charge. Women must apply for All Women Count through the state of South Dakota to receive the appropriate coverage.

Access to Health Screenings

Prevention is the best approach to managing your health. While many routine screenings aren’t readily available at our clinic, we work with patients to get their health care screenings – including helping patients find and fill out financial assistance paperwork.

Extended Hours: Coyote Clinic

This location also offers the University of South Dakota Coyote Clinic, a student-run health care service providing free medical care to underinsured patients. These medical students serve all ages under the supervision of local board-certified physicians and advanced practice providers.

  • Primary care, acute care and sports physicals every Tuesday, 5 – 6:30 PM
  • Walk-in appointments are welcome Tuesdays before 6 PM, appointments preferred
  • Psychiatric and behavioral health care first Wednesday of the month, 5 – 6 PM by appointment only
