Avera Granite Falls Care Center consists of two neighborhoods, each comprised of 24 resident rooms. Each living area is spacious and equipped with an extra-wide bed, wardrobe, dresser, night stand, TV, and bathroom with an extra-large shower.

Our Services
Rehabilitation Program
Our Care Center offers a full rehabilitation program for all residents. People often come for a short stay as they gain strength before they return home, while others may stay may be for an extended time.
Physical therapy, occupational therapy and speech therapy are used as the physician sees necessary for specific and intensive restorative therapy at the Care Center.
In addition, every resident is eligible for the Restorative Nursing Program. This program strives to maintain and improve your physical function. Individualized programs are created and implemented to meet the needs of each resident.
Social Services
Our Social Services Department provides assistance to our residents and their family or representatives to enhance and maintain their psychosocial well-being. Support groups and/or psychotherapy are available to our residents on a weekly basis.
Leisure Activities
Our Activities Department strives to help each resident to maintain their individual leisure pursuits. The Care Center offers worship services, intergenerational groups, outings, weekly happy hour with live music, parties for all special occasions and much more.
The Care Center is always involved in excellence programs to enhance and improve the care delivered to the residents we service. We strive to be responsive, considerate and innovative while providing excellent customer service to the people of our community.
