Billing & Insurance

As a patient, it's your responsibility to provide our Business Office with accurate insurance policy information. Please bring your most current insurance, Medicaid or Medicare card to each of your appointments. Patients with supplemental insurance also need to provide policy information to the Business Office.

Pay Your Bill Online

We will submit claims to your insurance company as a courtesy; however, the account balance is ultimately the patient's responsibility. It is also patients' responsibility to know what their policy covers and what it does not. If your insurance requires an office visit co-pay, this will be required at the time of your visit.
