AveraChart
AveraChart is an easy way to access your health care information, interact with your care team, and be a more active partner in your own health care. New users can register for AveraChart or log in if you already have an account. You may bookmark this page in your browser or smartphone favorites for easy future reference. If you have trouble creating a new account or logging in to your existing account, you can call the AveraChart Help Desk 24 hours a day, 7 days a week at 1-855-667-9704.
AveraChart features include:
- Message your care team or physician.
- Manage appointments.
- Request prescription refills.
- Read and review test and lab results, and your physician notes.
- See appointment times and request appointments with select providers.
- Easy access to find important information like vaccination history and medications.
AveraChart lets you do all of these things and more on your smartphone, tablet or computer.
Do not use AveraChart for urgent medical matters. If you are experiencing a medical emergency, call 911.

Please Note: You cannot create a virtual appointment with your provider from this page. You must contact your clinic and have them set the appointment for you.
Information Blocking
Learn about the information blocking regulation and sharing access to AveraChart with your family. With AveraChart, you get real-time access to medical record information. Your physician’s notes are also part of your chart. By offering more timely access to your health information, we believe you can form a more collaborative relationship between you and your care team, and be in control of your health care. Notice: Such timely sharing may also give you access to test results and documents even before your Avera care team contacts you to review. Read more about information blocking.
